OFFICIAL RULES - By entering this charity raffle, you are stating that you have read all of the raffle rules and understand them completely. If you have any questions about the rules below please call our office and we will do our best to answer all of your questions. 1. Purchaser must be at least 18 years of age at the time of entry and a legal resident of the United States to be eligible for the raffle. 2. Raffle starts on May 20, 2013 and ends August 30, 2013 at 11:00 p.m. online tickets will be sold until August 25, 2013 due to processing time or until tickets have been sold, whichever occurs first. 3.One (3) winners will be drawn at random on August 31, 2013 at 02:00 pm at the Calcutta Volunteer Fire Headquarters 15445 Pugh Rd, Calcutta, OH 4.Tickets are $20.00 each and no more than 2500 tickets will be sold. Payable by Cash / Certified Funds or by online Donations 1.If using online donations the raffle ticket will be filled out by CVFD at 15445 Pugh Rd in East Liverpool, OH and the receipt (ticket stub) will be mailed to purchaser as well as email confirmation of ticket number.
2. If using the online donation option the raffle ticket will be filled out with the purchaser information provided by the online donation company (PayPal). This information should match your government issued identification. 5. Upon purchase of raffle ticket, all sales are final and non-refundable. 6. Winner will take ownership of a 2013 Polaris Sportsman 500 H.O. to be obtained from the Calcutta Volunteer Fire Department. 7. Winner notification will be made by telephone and/or mail. 8. Winner must claim prize in person.  Winner is responsible for picking up and/or delivery of ATV.  9. The winner will need to provide proper government issued identification that matches the winning raffle ticket information to the satisfaction of The Calcutta Volunteer Fire Department. 10. Substitution or exchange of prize is not permitted.  No cash prize will be awarded - 2nd Place & 3rd Place winners will be issued a Visa Gift Card of CASH VALUE for that prize won.11.The Calcutta Volunteer Fire Department will file a form 1099 with the Internal Revenue Service to report the value of the prize won. The winner will also receive a copy of this 1099 form. 12. Information gathered from raffle ticket sales will not be sold or shared with any third party organization.
